THE GETTING STARTED STEPS:
1. Give us a call in order to establish what services you are looking for (it’s ok if you don’t know!) and your main concerns and any questions you have. If you prefer to begin with step 2, we will call you after we receive the Registration Form. We do not have a receptionist, so please leave us a message if we are unable to immediately answer your call

Out of Network Insurance and Private Pay Information

Moving Parts Therapy is an Out-of-Network Provider (you may be reimbursed for up to 80% of our fee) for other insurance companies.
​
You may have an out-of-network benefit with your insurance. This means that you would pay our fee at the time of service, and your insurance company would reimburse you directly. Your insurance plan may reimburse you for up to 80% of our fee. Call them to find out how much they will reimburse.
Our fees: Evaluation (60 minutes, includes detailed written document completed outside of the evaluation appointment) 370.00 Call for current session fee. We take cash, checks, credit cards (3% surcharge on credit card payments), debit cards and most HSA cards.

In Network Insurance Information

Anthem Blue Cross (we do not take Blue Cross HMO BadgerCare)
We ask that you contact your insurance to verify that we are contracted with your plan and to confirm coverage and benefits as your plan may have exclusions and/or prior authorization may be required. Please contact your insurance company for details.
When contacting your insurance company, it is important to verify the following information as it pertains to your insurance coverage:

To find out if you are eligible for in network insurance benefits for occupational therapy, call your insurance provider and ask:

Verify that we are contracted with your insurance plan.

Find out if prior authorization is required.

Deductible: This is portion of eligible medical expenses that you must pay before the insurance company will make any benefit payments.

Coinsurance: This is the percentage of coverage that the insurance company agrees to pay after your deducible is met.

Out of pocket maximum: This is the maximum amount that you will be required to pay, according to your insurance policy.

Ask if there a limit on the number of sessions that your policy allows.​
What To Bring To Your First Appointment

Physician Referral/prescription-Not required to receive therapy, however some insurances require it for payment. Check your insurance plan.

Moving Parts Therapy forms-Have you submitted the Pediatric History Form and the Registration Form?

Reports from other providers-Please bring a current IEP if your child has one. Feel free to also submit other reports that may be helpful. (i.e., psychology, medical)

Socks for all adults and children. Socks are required for health protection in the clinic.

Water bottle (No food, chewing gum or other drinks allowed inside clinic)

Covid 19 precautions- face masks are optional. If your child is unable to cover nose and mouth when sick and coughing and sneezing, they will need to wear a mask.

Please do not bring siblings to appointments if at all possible. If it is unavoidable, please discuss this with your therapist before hand. We do not have a waiting area. Children must be able to sit quietly in the gym with their parent. They may not be on the gym equipment.
3. Fill out the Pediatric History Form. For your privacy, we will send this to you in an encrypted email once we have received your Registration Form

4. Schedule the evaluation. We will email you the available times.
​
5. Attend evaluation appointment and discuss the results with the therapist.
​
6. Schedule treatment sessions if you want to do so. You can schedule these at the end of the evaluation session. You can also wait to schedule until you recieve the evaluation document. Attendance is twice per week. Pay for each session at time of serivce. Discontinue at any time.
